No I used without mods. Put on brown gorka, take AK+GP, switch to GP, press [mouse button right] to look through sights of GP , press [X] 1 time to crouch, press [W] to walk forward. Oh and be sure to hold aim on or above horizon because if you aim at the ground it is not visible.

I can confirm the Gorka issue. Gorka's top part of the hood clips through character's head.
In first person view, that top part of the hood is visible when moving forward while in crouched stance and aiming down the side grenade launcher sights (GP-25 and M320). It's only visible when you do that, probably because of the FOV which gets increased when aiming.
In third person view, Gorka issue is clearly visible especially when crouching.
Regarding that grenade in mid air, I wasn't able to reproduce.
